About the Role
The Clinical Practice Leader uses the nursing process to deliver patient- and family-centered care, valuing the uniqueness of each individual and addressing the physical, psychological, emotional, and social needs of the diverse patient population served by Jefferson Health. This role requires advanced preparation and demonstrates a high degree of professional competence in a specialized nursing field, applying a wide range of strategies in problem-solving situations while utilizing the nursing process. The Clinical Practice Leader promotes and supports evidence-based practice.
Responsibilities
- Interacts with co-workers, patients, their family/designated care partner(s), and other staff consistent with the values of Jefferson.
- Integrates evidence-based practice, clinical expertise, patient and family preferences, and cultural beliefs to deliver optimal healthcare.
- Partners with patients and their family/designated care partner(s) to identify learning needs, providing education in useful, understandable, and affirming ways.
- Minimizes risk of harm to patients and others by following established nursing practices, protocols, and individual performance standards.
- Uses information technology to communicate, manage knowledge, mitigate error, and support ethical decision-making.
- Applies leadership skills to coordinate and delegate aspects of care to team members based on their education, competence, and scope of practice.
- Assists staff nurses in utilizing the nursing process to provide professional nursing care based on established standards, policies, and procedures.
- Supports staff in implementing evidence-based practices to improve quality of care.
- Functions effectively within nursing and inter-professional teams, fostering open communication, mutual respect, and shared decision-making.
- Recognizes and respects patients' and their family/designated care partner(s)' preferences, priorities, needs, and cultural beliefs in providing safe, compassionate, and coordinated care.
- Creates an environment of belonging and inclusion, demonstrating problem-solving and conflict resolution skills.
- Facilitates orientation and continuous education of staff, including competency responsibilities.
- Responsible for Quality & Safety (Q&S) NSI responsibilities, including reviewing data and action plans.
- Collaborates with Clinical Nurse Specialists (CNS) and Clinical Nurse Educator Partners (CNEP).
- Promotes staff professional development.
- Demonstrates a commitment to learning and seeks opportunities for continued professional growth and development.
- Acts as a primary preceptor for appropriate staff members.
- Actively participates in hospital- and practice setting-based committees, including NPG and safety and quality improvement initiatives.
- Demonstrates willingness to give and receive feedback, integrating appropriate changes into practice.
- Serves as a Clinical Educator for the Honikman and Bala Cynwyd locations.
Requirements
- Graduate of an accredited school of nursing (Specialized Diploma required).
- 3 years of experience as a Registered Nurse (RN) in an acute care setting.
- At least 1 year of experience in Endoscopy.
- Patient- and Family-Centered Practice: Knowledge of how to convey respect and collaborate with patients, families, and designated care partners in care and care planning. Ability to educate and mentor staff in patient- and family-centered practices.
- Cultural Awareness: Knowledge of diverse backgrounds and ability to convey respect and connect effectively. Ability to educate and mentor staff in cultural awareness and practices.
- Effective Communication: Understanding of communication concepts, tools, and techniques. Ability to transmit, receive, and interpret ideas, information, and needs effectively. Uses partnership language with patients and families and delegates appropriately. Ability to educate and mentor staff in effective communication skills.
- Patient and Designated Care Partner Education: Knowledge of child and adult education techniques. Ability to elicit learning priorities and needs, and educate patients and their designated care partners on conditions, treatments, medications, and next steps in recovery. Ability to educate and mentor staff in these techniques.
- Medication Administration: Knowledge of medications used to treat patients and ability to apply this knowledge in administration. Ability to educate and mentor staff in medication administration techniques.
- Discharge Planning: Knowledge of post-discharge needs and ability to partner with patients and families in developing transition of care plans and arranging follow-up care. Ability to educate and mentor staff in discharge planning and transition of care processes.
- Assessment and Diagnosis: Knowledge of tools and techniques for patient assessment and nursing diagnosis or clinical judgment. Ability to explore and analyze symptoms and generate resolutions. Ability to educate and mentor staff in assessment and diagnosis techniques.
- Health Information Documentation: Knowledge of the relevance of health information and appropriate documentation methods. Ability to educate and mentor staff in health information documentation.
Qualifications
- ACLS - Advanced Cardiovascular Life Support (American Heart Association).
- BLS - Basic Life Support (American Heart Association).
- PALS - Pediatric Advanced Life Support (American Heart Association).
- RN - Licensed Registered Nurse (State of Pennsylvania or State of New Jersey).
Preferred Qualifications
- Master’s Degree in Nursing with emphasis in clinical nursing practice.
- CNE - Certified Nurse Educator (National League for Nursing).
Additional Requirements
Physical demands include the ability to lift and carry 40-50 lbs, push/pull patients on beds or stretchers (requiring 29 lbs. push force), and perform lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking is required. Patient care providers may also need to perform activities such as kneeling, bending, squatting, and performing CPR.
