About the Role
As a Clinical Pharmacy Technician specializing in Emergency Department (ED) Medication Reconciliation, you will collect clinical data related to patients' home medication therapy. This involves interviewing patients about their medications, verifying information through various software programs and retail pharmacies, and entering this data into the hospital's computer system for caregiver access. You will also perform general pharmacy technician duties, including medication preparation and distribution.
Responsibilities
- Interview patients regarding their home medication therapy and verify information using interactive software programs and retail pharmacies.
- Transfer prescription medication information into the hospital’s computer system for caregiver access.
- Perform data entry, report setup, and editing using the hospital’s computer software.
- Aseptically prepare IV admixtures in compliance with departmental, hospital, and licensing agency standards.
- Fill and distribute medications through first fill, cart fill, and/or Automated Drug Systems.
- Maintain functional usage of the hospital computer system and interfaced pharmacy systems.
- Familiarize yourself with generic and trade name medications.
- Perform narcotic distribution duties and automated dispensing medication refills.
- Fill oral prepackage syringes and operate Medication Prepackage Machines while adhering to federal prepackaging guidelines.
- Maintain active Tennessee State Pharmacy License and National Pharmacy Technician Certification.
- Stay familiar with JCAHO/DNV Standards for the Department of Pharmacy and comply with unit dose medication distribution practices.
- Perform all duties with excellent customer relation skills.
Requirements
- High School Graduate (required).
- Certified Pharmacy Technician (CPhT) (required).
- Proficiency in Microsoft Office (Excel, Word) (required).
- Minimum of one year of pharmacy experience (required for CPhT).
- LPN or RN with CPhT Certification accepted with no experience.
- Nationally Certified Pharmacy Technician (required).
- Active Certified Pharmacy Technician with the ability to obtain a Tennessee State Pharmacy License within 60 days of employment (required).
Preferred Qualifications
- Associate’s Degree in Pharmacy Technology or completion of an accredited Pharmacy Tech course.
- One year of IV compounding experience.
