Clinical Pharmacy Specialist - PRN - AAMC

Position Objective:

  • Carries out Clinical Pharmacy services through promoting evidence-based pharmacotherapy and sound pharmaceutical care.
  • Develops and oversees medication management protocols and drug therapy policies.
  • Serves as a Clinical Pharmacy resource for the health system.

Essential Job Duties

  • Conducts departmental activities to meet the goals, objectives and policies of the pharmacy department.
  • Participates in improvements in interdepartmental policies and procedures.
  • Maintains accurate records as required by departmental policies and procedures and/or federal and state board regulations.
  • Completes all core competencies and documents clinical activities accurately and according to policies and procedures.
  • Serves as the medication expert on medical and surgical units.
  • Provides pharmaceutical care, dosing and guideline recommendations.
  • Completes pharmacotherapy drug class reviews, formulary addition/deletion, assimilation of evidence-based protocol/order-sets, medication safety related activities and/or targeted medication expense reductions.
  • Aids in appropriate transition of care services, including medication reconciliation and discharge education.
  • Serves as the pharmacy representative on multidisciplinary committees.
  • Edits other healthcare providers on approved treatment guidelines and criteria.
  • Serves as a resource for Pharmacy School Students and AAMC Pharmacy Residents.

Requirements

  • Doctor of Pharmacy degree required.
  • Completion of PGY-1 residency or 2 years of demonstrated clinical experience required.
  • Completion of PGY-2 residency or board certification in pharmacotherapy preferred.
  • Current Maryland State License required.
