Clinical Pharmacy Specialist-Internal Medicine

Description

• Participates in daily patient care rounds.
• Maintains and updates patient profiles with demographics, allergies, and current medications on a daily basis.
• Promotes the use of the formulary by converting non-formulary orders to formulary, when possible, and coordinates procurement of non-formulary drugs when necessary.
• Is competent in the range of treatment needed by the following patient as appropriate to the ages (Adult, pediatric, geriatric, adolescent, and psychiatric).
• Prevents, detects, monitors, documents, and reports adverse drug reactions and medication errors.
• Performs pharmacokinetics, IV to PO conversions, therapeutic interchanges, and other clinical pharmacy functions accurately and in a timely manner.
• Responds to medication related inquiries from other health care professionals and patients.
• Documents clinical interventions and cost avoidance as part of the departmental documentation program.
• Serves as a consultant to hospital Pharmacy & Therapeutics committee and other hospital sponsored clinical and performance improvement committees.
• Participates in the planning and implementation of system-wide pharmaceutical care programs to optimize drug therapy outcomes.
• Completes one clinical project per fiscal year.

JOB SPECIFICATIONS

• Required Knowledge, Skills and Abilities:
  • Knowledge: Pharm.D. or equivalent from an ACPE-accredited School of Pharmacy and a current pharmacist license in good standing with appropriate State Board of Pharmacy. Residency in an ASHP accredited program preferred. Pharmacotherapy board certification recommended.
  • Experience using computer applications including word processing, spreadsheets, and pharmacy information systems.
  • Skills: Thorough knowledge of state and federal laws and regulations governing pharmacy services. Proficient in the use and application of new medications and technology. Effective verbal and written communication skills are required. Ability to effectively participate on an interdisciplinary team or committee. Knowledge of pathophysiology, interpretation of diagnostic criteria, pharmacokinetics and pharmacotherapeutics. Problem solving ability and analytical thinking preferred.
• Abilities: Ability to work independently and handle stress appropriately. Ability to work cooperatively with other members of Mercy Hospital and Medical Center and pharmacy staff. Ability to handle multiple tasks and adapt to changes in workload and work schedule. Ability to set priorities, make critical decisions, and respond to client concerns. Fosters a team environment. Ability to communicate effectively (verbal and written).
