Duties and Responsibilities
- Participates in daily patient care rounds.
- Maintains and updates patient profiles with demographics, allergies, and current medications on a daily basis.
- Promotes the use of the formulary by converting non-formulary orders to formulary, when possible, and coordinates procurement of non-formulary drugs when necessary.
- Ensures competence in treating various age groups (Adult, pediatric, geriatric, adolescent, and psychiatric).
- Monitors drug therapy for appropriateness and safety, and reports adverse drug reactions and medication errors.
- Performs pharmacokinetics, IV to PO conversions, therapeutic interchanges, and other clinical pharmacy functions accurately and in a timely manner.
- Responds to medication-related inquiries from other healthcare professionals and patients, documenting clinical interventions and cost avoidance as part of the departmental documentation program.
- Serves as a consultant to the Pharmacy & Therapeutics committee and other hospital-sponsored clinical and performance improvement committees.
- Contributes to newsletters, in-services, and educational materials for pharmacy staff and other healthcare professionals.
- Plays a role in planning and implementing system-wide pharmaceutical care programs to optimize drug therapy outcomes.
- Completes one clinical project per fiscal year, collaborating with the pharmacy clinical team.
- Ensures all patient care provided meets or exceeds all regulations, statutes, and HFAP standards.
- Reports daily to the Clinical Pharmacy Manager on overall operations, projects assigned, and expectations of the role.
- Counsels patients on discharge medications (e.g., warfarin education as necessary) and coordinates medications to beds and interventions with the Outpatient Pharmacy when needed.
- Rotates shifts AM&PM.
Job Specifications
- Required Knowledge, Skills and Abilities:
- Knowledge: Pharm.D. or equivalent from an ACPE-accredited School of Pharmacy and a current pharmacist license in good standing with appropriate State Board of Pharmacy. Residency in an ASHP accredited program preferred. Pharmacotherapy board certification recommended.
- Experience: Knowledge of state and federal laws and regulations governing pharmacy services. Proficient in the use and application of new medications and technology. Effective verbal and written communication skills are required. Ability to effectively participate on an interdisciplinary team or committee. Knowledge of pathophysiology, interpretation of diagnostic criteria, pharmacokinetics and pharmacotherapeutics. Problem-solving ability and analytical thinking preferred.
- Abilities: Ability to work independently and handle stress appropriately. Ability to work cooperatively with other members of Mercy Hospital and Medical Center and pharmacy staff. Ability to handle multiple tasks and adapt to changes in workload and work schedule. Ability to set priorities, make critical decisions, and respond to client concerns. Fosters a team environment. Ability to communicate effectively (verbal and written).
