Full-time Clinical Pharmacy Specialist role at AHN Forbes Hospital, a 300+ bed community hospital serving the eastern suburbs of Pittsburgh. The pharmacist will also serve as a member of the antimicrobial subcommittee for the health network, collaborating with a large team of ID physicians and pharmacists on network initiatives.
Responsibilities
- Serve as the pharmacy expert for Infectious Diseases and Antimicrobial Stewardship, providing consultation and evidence-based recommendations.
- Collaborate with ID physicians and multidisciplinary teams to optimize antimicrobial therapy and improve patient outcomes via prospective audit and feedback, stewardship recommendations, and surveillance activities.
- Develop, implement, and maintain antimicrobial treatment guidelines, protocols, clinical pathways, and stewardship initiatives.
- Participate in quality improvement, medication use evaluations, outcomes research, and regulatory readiness activities.
- Educate pharmacists, physicians, nurses, and other healthcare professionals on infectious diseases pharmacotherapy and antimicrobial stewardship best practices.
- Serve as a preceptor for pharmacy residents and pharmacy students.
- Participate in local Pharmacy and Therapeutics committees, network antimicrobial subcommittee, and hospital-wide initiatives to promote medication safety, quality, and evidence-based patient care.
Qualifications
- PGY2 Infectious Diseases Pharmacy Residency preferred, or
- PGY1 Pharmacy Residency with at least four years of infectious diseases and/or antimicrobial stewardship experience in an acute care setting.
