About the role

This is a position for a second antimicrobial stewardship pharmacist. Daily responsibilities include prospective audit and feedback of restricted antimicrobial agents, stewardship review of patients with bacteremia, ICU handshake stewardship, and serving as an ID drug information resource for ID physicians, other providers, and other pharmacists.

  • Participates in education, quality improvement projects, and committee involvement at both the facility and system levels.

Requirements

The schedule for this position is Monday - Friday 8am - 4pm.

Qualifications

  • A Doctorate in Pharmacy degree and at minimum completion of a PGY1 residency program.
  • Without residency training a minimum of 3 years of health-system progressive clinical pharmacy experience (evaluating disease state & medication management e.g. thromboembolic, diabetes, oncology, pharmacokinetics, pain management & nutrition, investigational studies, etc.).

Skills

  • Critical evaluation of disease state management and medication management.
  • Ensuring appropriate and cost-effective medication therapy.
  • Drug information expertise in area of specialty.
  • Education of healthcare team, pharmacy, patients, and community.
  • Evaluation of medication orders for appropriateness and completeness.
  • Continuous quality improvement consistent with job expectations.
  • Development of innovative drug-related patient care practices.
  • Serving as a resource and mentor to other pharmacists, students, residents, and other health-care providers.
