Responsibilities

• Collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients.

Requirements

• Bachelor's Degree in PHARMACY from an accredited School of Pharmacy or PharmD in PHARMACY from an accredited School of Pharmacy.
• Coursework/Training: Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency or equivalent experience or equivalent education/experience.
• Upon hire: Oregon Pharmacist License.
• Within 2 years of hire Certification from Board of Pharmacy Specialties.
• 2 years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting - OR - FIVE years of pharmacy practice experience with at least 2 years of experience in relevant setting in lieu of PGY-1 and PGY-2 residency requirement.

• Upon hire: Board of Pharmacy Preceptor license when eligible and where applicable.

Pay

Pay range: $83.92 – $134.17 (base pay). Additional compensation may be available for this role, such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.

Schedule

Full‑time, Day shift.
