Clinical Pharmacy Specialist

We are seeking a Clinical Pharmacist to collaborate with healthcare professionals in providing safe, effective, and patient-centered medication therapy. Key Responsibilities Include Reviewing patient records to ensure appropriate medication use and developing optimized medication plans.Consulting with prescribers, nurses, and patients regarding drug therapy, dosing, and administration.Serving as a clinical resource and participating in patient assessments and medication therapy services.Collaborating with healthcare teams, mentoring students, and delivering educational sessions.Evaluating medication orders, supporting quality assurance, and ensuring regulatory compliance.Utilizing pharmacy information systems and automation tools for clinical documentation and order verification. Qualifications Include Bachelor of Science in Pharmacy or Doctor of Pharmacy from an accredited program.Current pharmacist license in the practice state.Preferred: two or more years of clinical pharmacy experience or completion of a PGY‑1 residency. The role emphasizes professionalism, teamwork, and adherence to safety and confidentiality standards, contributing to high-quality pharmaceutical care in a hospital setting.
