Clinical Pharmacy Specialist

Responsibilities

• Provides specialized clinical activities relating to designated clinical programs.
• Performs medication utilization activities such as anticoagulation pharmacokinetics, dose adjustments, order verification, and IV to PO conversions.
• Participates in committees and represents the Department of Pharmacy within the hospital or ambulatory settings.

Requirements

• Three years of experience required.
• Doctorate in Pharmacy.
• Specialization in Pharmacy.
