Clinical Pharmacy Specialist - Adult Outpatient Specialty Clinics Pharmacist

This ambulatory pharmacist will work with other pharmacists in various adult specialty clinics such as Neurology, Transplant, Hepatology, and Virology to provide pharmacy services to patients. Common patient populations include Multiple Sclerosis, Migraine, Movement Disorders, Epilepsy, Solid Organ Transplant, Hepatitis C, and HIV.

Responsibilities

• Participate in the provision of pharmaceutical care in select clinics, including:
  • Being present in clinics when patients are scheduled to see the clinical pharmacist
  • Completing comprehensive patient assessments (medication history, physical assessment, laboratory values, patient interview, past medical history)
  • Working collaboratively with physicians, nurses, pharmacists, and other healthcare providers as part of a multidisciplinary team
  • Providing disease state management and medication therapy management through collaborative practice agreements and protocols
  • Delivering in-depth patient medication assessment and education
  • Following up on therapeutic recommendations via telephone calls and return clinic appointments
  • Identifying, evaluating, and preventing adverse drug events
  • Communicating therapeutic recommendations to healthcare providers
  • Documenting clinical information in the electronic medical record
  • Coordinating medication-planning activities (prescription facilitation, patient education, coordination with family/home health providers)
  • Addressing financial issues to ensure patients receive needed medications
  • Providing drug information and education to healthcare professionals
  • Serving as a liaison for ambulatory pharmacy staff regarding patient medication therapy
  • Evaluating patient-specific medication regimens for appropriateness of indication, dose, route, interactions, efficacy, adherence, organ dysfunction adjustments, adverse reactions, misuse potential, cost-effectiveness, and formulary availability
• Provide staffing support in Specialty Pharmacy Services, including completing patient follow-up calls to address side effects and adherence.
• Serve as a role-model and educator for Doctor of Pharmacy students and pharmacy residents by:
  • Developing goals and objectives for residency programs and advanced/introductory pharmacy practice experiences
  • Preparing and administering student/resident rotation evaluations
  • Participating in formal experiential training and didactic teaching for residency programs and pharmacy practice experiences
  • Participating in recruiting activities for pharmacy residency programs
• Provide daily communication and follow-up to clinical pharmacists, pharmacy areas, and other healthcare professionals.
• Deliver in-service education to pharmacists and healthcare team members as needed.
• Assist with medication questions, access, and continuity of care for physicians, nurses, team members, and patients.
• Document patient care activities in compliance with accreditation standards, including:
  • Clinical services and patient care in the electronic medical record
  • Patient assessment and therapeutic plan recommendations
  • Medication errors and adverse drug reactions in standard reporting systems
  • Patient allergies, height/weight information, medication counseling, interventions, clinic encounters, and follow-up calls
• Develop and provide new department clinical service initiatives, including:
  • Participating in on-call rotation (approximately 1-2 weeks per year) for drug therapy issue problem-solving
  • Developing or contributing to protocols for specialty medication prescribing, dispensing, and monitoring
  • Providing leadership in department or interdisciplinary hospital committees on drug therapy issues
  • Developing drug information resources (e.g., pocket cards, practice guidelines) to foster cost-effective prescribing
  • Designing, implementing, and coordinating clinical components of pharmacy or hospital-wide information systems
• Administrative responsibilities:
  • Coordinating therapeutic decision-making for complex or literature-sparse scenarios
  • Serving as a primary reference for new drug therapy advances in specialized practice areas
  • Identifying and resolving significant/unusual patient drug therapy problems
  • Completing clinically oriented departmental goals
  • Developing and coordinating policies, procedures, and clinical reference documents for new therapies
  • Managing and analyzing medication error events, including participation in sentinel event review teams
  • Interviewing and evaluating prospective pharmacy practitioners
  • Adhering to department policies and procedures
  • Contributing to the department budgetary process by reviewing and designing budget initiatives
  • Participating in the implementation and monitoring of new programs (e.g., P&T Committee initiatives)
• Quality improvement:
  • Providing leadership in university committees on drug therapy
  • Developing and conducting measurement activities to demonstrate Specialty Pharmacy Service value (e.g., adherence and outcome measures)
  • Performing quality assurance monitoring of Specialty Pharmacy Services
  • Developing educational programs to certify clinical competence of pharmacists and improve technician understanding of medical conditions
• Other duties:
  • Conducting research in area of expertise and presenting/publishing results at professional meetings or journals

  • Maintaining compliance with all competencies and educational requirements

Requirements

• Bachelor of Science in Pharmacy or Doctor of Pharmacy Degree
• Eligibility for or current licensure by the Iowa Board of Pharmacy Examiners (required prior to start)
• Completion of an ASHP-accredited PGY1 pharmacy practice residency or 2 years’ experience in a complex multidisciplinary healthcare environment
• Excellent written and verbal communication skills

Qualifications

• Satisfactory completion of an ASHP-accredited PGY2 residency in Ambulatory Care (highly desirable)
• Employment in an outpatient clinic environment (highly desirable)
• Active board certification in an applicable Pharmacy Specialty (highly desirable)
• Active board certification in any Pharmacy Specialty (desirable)
• Ability to work independently, prioritize multiple stakeholders/tasks, manage details, and deliver results in a demanding environment
• Strong interpersonal skills and demonstrated ability to work collaboratively

Schedule

• Full-time
• Hybrid work within Iowa (eligible upon completion of orientation; work arrangement form required at start of employment)
• Work arrangements reviewed annually and must comply with remote work program, related policies, and employee travel policy
