About the role

The Ambulatory Care Clinical Pharmacy Manager at MSHS reports to the Senior Director, Ambulatory Pharmacy. This role focuses on delivering clinical pharmacy services with a focus on chronic disease management, promoting clinically rational drug therapy, and managing medication management performance improvement initiatives.

Responsibilities

  • Delivery of clinical pharmacy services with a focus on chronic disease management

  • Identify, develop, and implement new pharmacy strategies and innovation pilot projects across responsible clinics/areas

  • Precepting for the MSHS PGY1 and MSHP PGY2 ambulatory care pharmacy residency programs and students from colleges of pharmacy

  • Manage pharmacy students and residents as requested

  • Supervise pharmacy employees

  • Support financial, quality and utilization analytics and performance improvement evaluations

  • Interview candidates and support in leading orientation activities

Qualifications

  • Doctor of Pharmacy Degree required

  • ASHP-Accredited PGY-1 required; PGY-2 preferred

  • Board Certification (BCACP) or obtain BCACP within 2 years of hire, required

  • Licensing and Certification Requirements: Registered Pharmacist, NYS Board of Pharmacy, continues to meet regulatory, educational, and licensure requirements throughout employment, as set forth by New York State regulations

  • Additional Skills and Qualities: Chronic disease management, care management, MTM experience, CDTM experience, pop-health experience
