Clinical Pharmacist WDH

About the role

This role involves rotating days and evenings in the Emergency Department (ED) and on inpatient units. The pharmacist will be responsible for various emergency medicine pharmacy activities such as code response, rapid sequence induction (RSI) response, pharmacy-driven protocols, provider education, drug distribution for EMS services, and medication reconciliation.

Responsibilities

• Optimize drug therapy by identifying, resolving, and preventing drug-related problems and improving therapeutic outcomes.
• Provide patient care according to laws and regulations governing pharmacists and the practice of pharmacy.
• May prescribe medications under specific collaborative drug therapy management agreements (CDTM).

Qualifications

• Doctor of Pharmacy (PharmD) degree is required.
• No experience in lieu of a degree is accepted.
• Licenses and credentials include a valid pharmacist license in New Hampshire.
• Extensive knowledge and ability in all areas of the department, including IV admixtures, drug information, ambulatory, manufacturing, and drug distribution.
• Attention to detail and ability to assess patient data relative to age-specific needs.
• Knowledge of pharmacy practice settings, legal requirements, prescription information, calculations, drug identification, and generic equivalents.
• Familiarity with proper drug handling and storage practices.
