Job Summary

This is a PRN position.

Qualifications

  • B.S. in Pharmacy from an ACPE-accredited School of Pharmacy OR a Doctor of Pharmacy from an ACPE-accredited college of pharmacy
  • Minimum of 3 years experience as an Advanced Clinical Pharmacist (in a specific area of focus) or at least 5 years of general hospital pharmacy experience required, preferably within BMT
  • Completion of a PGY2 pharmacy residency or fellowship - AND - BCPS certification or other Board of Pharmacy Specialty (BPS) certification

Responsibilities

  • Interprets orders and transcribes or verifies them accurately and efficiently into the pharmacy computer system
  • Accurately compound and dispense pharmaceuticals
  • Complies with facility policy and procedures and regulatory requirements
  • Maintains the hospital formulary, minimizing nonformulary procurements, utilizing therapeutic interchanges and promoting rational drug therapy selection
  • Attends and actively participates in daily multidisciplinary and bedside rounds
  • Completes drug utilization reviews to promote rational drug therapy and implement improvements identified
  • Provides accurate and timely medication information to healthcare staff and patients
  • Develops and manages antimicrobial management program (AMP)
  • Documents all clinical activities and interventions accurately and completely
  • Mentors and educates pharmacy staff and precepts students/residents
  • Supports and participates in the goals of the organization’s quality improvement, risk management and patient safety program
  • Participates in the development and review of pharmacy policies and procedures

Requirements

  • Obtains and maintains BLS-ACLS or PALS current certification as appropriate
  • Maintains appropriate controlled substance records
  • Works with the Pharmacy Director and appropriate manager to identify and develop processes to meet or exceed all controlled substance diversion prevention and monitoring standards
  • Participates in quality and regulatory reviews and audits
  • Prioritizes work and lead others to comply with facility medication turn-around time policy and procedure
  • Identifies areas for improved efficiency
  • Assists in implementation of process changes
  • Supervises and assists pharmacy technicians and other support staff effectively
  • Participates in establishing pharmacy goals and objectives and assists in the implementation
  • Participates with various nursing and medical staff committees as necessary
  • Assists with staff performance evaluations
  • Acts as manager in charge as assigned
  • Provides review and guidance on physician order sets and other evidenced based medication use guidelines
  • Assists with the preparation of the pharmacy newsletter and/or other forms of written and electronic communication
  • Participates in all aspects of the drug formulary management process
  • Assists with the development, implementation, and monitoring of pharmacy operations initiatives (operational/clinical)
