Job Summary
This is a PRN position.
Qualifications
- B.S. in Pharmacy from an ACPE-accredited School of Pharmacy OR a Doctor of Pharmacy from an ACPE-accredited college of pharmacy
- Minimum of 3 years experience as an Advanced Clinical Pharmacist (in a specific area of focus) or at least 5 years of general hospital pharmacy experience required, preferably within BMT
- Completion of a PGY2 pharmacy residency or fellowship - AND - BCPS certification or other Board of Pharmacy Specialty (BPS) certification
Responsibilities
- Interprets orders and transcribes or verifies them accurately and efficiently into the pharmacy computer system
- Accurately compound and dispense pharmaceuticals
- Complies with facility policy and procedures and regulatory requirements
- Maintains the hospital formulary, minimizing nonformulary procurements, utilizing therapeutic interchanges and promoting rational drug therapy selection
- Attends and actively participates in daily multidisciplinary and bedside rounds
- Completes drug utilization reviews to promote rational drug therapy and implement improvements identified
- Provides accurate and timely medication information to healthcare staff and patients
- Develops and manages antimicrobial management program (AMP)
- Documents all clinical activities and interventions accurately and completely
- Mentors and educates pharmacy staff and precepts students/residents
- Supports and participates in the goals of the organization’s quality improvement, risk management and patient safety program
- Participates in the development and review of pharmacy policies and procedures
Requirements
- Obtains and maintains BLS-ACLS or PALS current certification as appropriate
- Maintains appropriate controlled substance records
- Works with the Pharmacy Director and appropriate manager to identify and develop processes to meet or exceed all controlled substance diversion prevention and monitoring standards
- Participates in quality and regulatory reviews and audits
- Prioritizes work and lead others to comply with facility medication turn-around time policy and procedure
- Identifies areas for improved efficiency
- Assists in implementation of process changes
- Supervises and assists pharmacy technicians and other support staff effectively
- Participates in establishing pharmacy goals and objectives and assists in the implementation
- Participates with various nursing and medical staff committees as necessary
- Assists with staff performance evaluations
- Acts as manager in charge as assigned
- Provides review and guidance on physician order sets and other evidenced based medication use guidelines
- Assists with the preparation of the pharmacy newsletter and/or other forms of written and electronic communication
- Participates in all aspects of the drug formulary management process
- Assists with the development, implementation, and monitoring of pharmacy operations initiatives (operational/clinical)
