Clinical Pharmacist, Population Health (remote)

About the role

Reviews, prepares and dispenses medication orders per physician order or prescription according to established policies, procedures, and protocols. Can work independently but stays within departmental policies and procedures.

Responsibilities

• Ensures safe, appropriate, cost effective medication therapies for patients according to established policies, procedures and protocols.
• Contributes to the quality and effective operation of the department.
• Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to physicians, as needed.
• Provides drug information and counseling to patients, families, and other healthcare professionals.
• Participate in the training, education, precepting, and evaluation of pharmacy students and/or residents as assigned.
• Provides work direction to pharmacy support personnel, verifies the daily activities of pharmacy technicians.
• Performs other related duties as assigned.

Qualifications

Education: Minimum Five year bachelor of science degree in pharmacy from an accredited college of pharmacy. Preferred Advanced training in at least one specialty clinical area.

Credentials/Licensure: Minimum Current registered pharmacist license and controlled substance license issued by the Michigan State Board of Pharmacy. Basic Cardiac Life Support (BCLS) certification may be required (at discretion of manager).

Related Experience: Minimum N/A Preferred One year of experience in a hospital or retail pharmacy.

Skills

Knowledge of drug regimen review, pharmacy software for order entry, allergy screening, drug interactions, and other related topics. Ability to communicate effectively. Basic knowledge of Word, Excel and other computer applications. Familiarity with standard desktop and windows based computer system, including email, e-learning, intranet and computer navigation. Ability to use other software required to perform essential functions.
