Clinical Pharmacist - Pharmacy

Responsibilities

• Interview and counsel patients on the use, storage, cautions and side effects of all medications prescribed.
• Provide and document patient education.
• Evaluate medication regimens for appropriateness, drug interactions, therapeutic duplications, non-compliance, adverse drug reactions, and cost effectiveness.
• Analyze medication regimens to ensure the most effective, least toxic, and most economical treatment plan.
• Document all clinical activities, update problem lists, and record information in the electronic health record as required.
• Maintain pharmacy records as required by law.
• Provide ongoing, comprehensive chronic disease management.
• Perform departmental administrative functions, committee involvement, quality assurance activities, statistical analysis, CME presentations, in-services, competency and proficiency reviews, and involvement in performance improvement groups.
• Operate under collaborative practice agreements as approved by the Wyoming Board of Pharmacy, which may include limited prescribing in defined situations.
• Ability to refer and consult appropriately.
• Assess, develop, and implement care plans, evaluate the effectiveness of treatment, and modify treatment as needed.
• Review the medical record for therapeutic effectiveness and appropriateness, including diagnosis, treatment, patient allergies, and drug interaction screening.
• Manage interventions and therapy between primary physician visits.
• Select appropriate medications for disease state management, monitor patient outcomes, analyze adverse drug events, and perform medication reconciliation.
• Manage formulary, quality assurance, medication utilization review, and staff development activities.

Qualifications

• Experience in managing medication therapy through direct patient assessment.
• Knowledge of patient counseling and education on medication use.
• Ability to evaluate medication regimens for appropriateness and cost-effectiveness.
• Proficiency in analyzing medication regimens to ensure therapeutic effectiveness and safety.
• Strong documentation skills to maintain accurate and up-to-date electronic health records.
• Experience in providing comprehensive chronic disease management.
• Skills in performing departmental administrative functions, committee involvement, quality assurance, and statistical analysis.
• Ability to present CME sessions and conduct in-services.
• Experience in competency and proficiency reviews and involvement in performance improvement groups.
• Knowledge of collaborative practice agreements and ability to operate under them.

• Experience in assessing, developing, implementing, evaluating, and modifying care plans.
• Knowledge of reviewing medical records for therapeutic effectiveness and appropriateness.
