Clinical Pharmacist - Per Diem Variable
About the role
The Clinical Pharmacist provides high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life.
Responsibilities
- Provide clinical pharmacy services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life.
- Collaborate with physicians, nurses, and other healthcare providers to ensure appropriate medication use and patient safety.
- Participate in medication reconciliation and medication therapy management activities.
- Assist in the development and implementation of policies and procedures related to pharmacy operations.
- Contribute to the education of healthcare professionals and patients regarding safe and effective medication use.
Requirements
- PharmD in Pharmacy, OR Bachelor's degree in pharmacy
- Upon hire: Oregon Pharmacist License
Preferred Qualifications
- Acute care hospital pharmacy experience, 1 year or equivalent experience
- Completion of ASHP-accredited PGY1 residency

Pay
The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
