Clinical Pharmacist - Part-Time

Option Care Health, Inc. is the largest independent home and alternate site infusion services provider in the United States. As a Clinical Pharmacist – Part-Time, you will provide safe and appropriate pharmacy services to patients in accordance with company policies, professional standards, and regulatory requirements. You will coordinate patient services with the Nursing Department, monitor patient progress, and educate patients, caregivers, and professionals on appropriate medication use. Responsibilities

• Determine the suitability of individual patients for home care by collecting and organizing patient-specific information and assessing their medical condition and prescribed medication therapy.
• Design, recommend, monitor, and evaluate patient-specific pharmacy care plans for home care patients, including making modifications to prescribed medication therapy or monitoring parameters as needed.
• Collect and organize patient-specific information to prevent, detect, and resolve medication-related problems, and identify appropriate monitoring parameters for each patient.
• Complete nutritional assessments and make appropriate recommendations.
• Modify pharmacy care plans based on evaluation of monitoring data and other pertinent patient-specific information, demonstrating competency in pharmacokinetic principles and assessing laboratory values.
• Provide medication-use education to Option Care staff, other professionals, patients, and caregivers using effective patient education techniques.
• Ensure continuity of pharmaceutical care to and from the home and other patient-care settings by systematically communicating pertinent patient information.
• Prepare and dispense medications using appropriate techniques, ensuring proper concentration, compatibility, stability, and storage while adhering to safety, quality assurance, and labeling policies.
• Comply with state/federal laws and USP standards, including DEA controlled substance regulations and perpetual inventory systems for schedule II narcotics and other medications subject to diversion.
• Manage the use, maintenance, and troubleshooting of medication administration equipment and medication-related equipment, including programming, advising on use, and reporting discrepancies or errors.
• Provide concise, applicable, and timely responses to drug information requests from healthcare providers and patients, formulating responses based on literature analysis.
• Attend educational seminars or participate in continuing education opportunities to maintain professional competency and comply with state-mandated requirements.
• Manage the use of investigational drug products according to established regulations and Option Care policies.
• Utilize pharmacy support personnel effectively by prioritizing workload, organizing workflow, and ensuring accuracy in their work while complying with accreditation, legal, regulatory, and safety requirements.
• Maintain confidentiality of patient and proprietary information, adhering to legal and ethical guidelines.

Requirements

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following:
  • At least one year of experience in sterile IV admixture procedures, or
  • Knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM), or
  • Completion of an ASHP accredited Pharmacy Residency Program, or
  • Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy, or
  • Equivalent experience as approved by the Vice President (VP) of Pharmacy and/or Sr. Vice President (SVP) of Clinical Services.
• Access to a reliable means of transportation for home visits, including a valid driver’s license and proof of insurance if using a personal vehicle.
• Experience providing customer service to internal and external customers, including meeting quality standards and evaluating customer satisfaction.
• Basic level skills in Microsoft Word, Excel, and PowerPoint.
• Willingness to travel up to 5% of the time for business purposes (within state).

Preferred Qualifications

• Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
• Experience in home care, hospital, or other clean room settings.
• At least 1 year of experience in indirect management of team members, including assisting in development, training, and assignment of work/projects.

Pay

Salary to be determined by the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Pay range is $49.14-$81.91 per hour.
