Clinical Pharmacist - Part-Time

About the role

The Clinical Pharmacist – PT, PD, CA is an entry-level position within the practice of clinical pharmacy in the alternate setting. This role involves providing safe and appropriate pharmacy services to Option Care patients, coordinating patient services with the Nursing Department, and educating patients, caregivers, staff, and other professionals about medication use.

Responsibilities

• Determines the suitability of individual patients for home care.
• Collects and organizes all patient-specific information needed to determine the suitability of patients for home care.
• Assesses patients' suitability for home care in accordance with Option Care policies.
• Designs, recommends, monitors, and evaluates patient-specific pharmacy care plans for the pharmacotherapy of home care patients.
• Prepares and dispenses medications using appropriate techniques and following Option Care policies and procedures.
• Ensures continuity of pharmaceutical care to and from the home and other patient-care settings.
• Provides medication-use education to Option Care staff, other professionals, home care patients, and/or their caregivers.
• Manages the use, maintenance, and troubleshooting of medication administration equipment and medication-related equipment used in the management of home care patients.
• Attends educational seminars or participates in other CEU opportunities to promote professional competency and complies with state mandated CEU requirements.
• Maintains confidentiality of patient and proprietary information.

Requirements

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following: at least one year of experience in sterile IV admixture procedures, knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM), completion of an ASHP accredited Pharmacy Residency Program, completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy, or equivalent experience as approved by the VP of Pharmacy and/or SVP of Clinical Services.
• Access to a reliable means of transportation which will enable the incumbents to make home visits to treat/attend to their patients.
• Experience providing customer service to internal and external customers, including meeting quality standards for services, and evaluation of customer satisfaction.
• Basic level skill in Microsoft Word, Microsoft Excel, and Microsoft PowerPoint.

Qualifications
