Clinical Pharmacist Inpatient - 7 on 7 off Overnights

Summary

Reports to the Pharmacist-in-Charge. Under general supervision of a Pharmacy Supervisor, responsible for interpreting, evaluating, preparing, and dispensing medications.

Responsibilities

• Knows and acts in accordance with the principles of the Brown University Health Corporate Compliance Program and Code of Conduct.
• Receives patient medication orders as written by a physician or practitioner. Reviews and interprets each order to determine completeness, therapeutic appropriateness, and safety.
• Contacts prescribers for interventions to ensure therapeutic efficacy, minimize risks, and optimize cost-effectiveness.
• Applies pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective use of medications.
• Provides drug information and consultations to prescribers, nurses, and other health care professionals.
• Supervises pharmacy interns and technicians, ensuring performance standards are met and work complies with regulations.
• Identifies, documents, resolves, monitors, and prevents drug therapy issues.
• Engages with clinical pharmacists or specialists for complex cases.
• Implements patient-focused medication therapy management and pharmaceutical care.
• Monitors and adjusts dosages and conducts laboratory monitoring for selected medications.
• Documents and reports clinical interventions, medication errors, adverse drug events, and near misses.
• Attends and participates in medical, multi-disciplinary, and specialty rounds.
• Reviews and approves formulations for sterile products requiring compounding or manipulation.
• Completes in-process and final checks for compounded medications.
• Trains healthcare practitioners on medication use.
• Serves as a preceptor for pharmacy residents and students.
• Participates in quality improvement activities and reporting programs.
• Coordinates transitions of care, as appropriate.
• Maintains professional competence through education and participation in professional organizations.

Qualifications

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy.
• Prior to start date, possess current unrestricted licensure as a licensed pharmacist or as a licensed 90-day temporary pharmacist in the State of Rhode Island. Obtain full licensure prior to expiration of 90-day temporary pharmacist license.
• Must possess and maintain current American Heart Association Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) Certifications within 120 days of hire.
• Successful completion of Post Graduate Year 1 (PGY-1) Residency from an accredited program and/or national Board Certification in Pharmacotherapy preferred.
• Post Graduate Year 2 (PGY-2) Residency from an accredited program in area of specialty strongly preferred, if applicable.
• Experience with MS Word, Excel, Outlook, and web browsers.
