About the role

The Clinical Pharmacist provides optimal pharmaceutical care on a daily basis, developing, implementing, and maintaining systems that promote desirable patient outcomes through safe and cost-effective drug therapies.

This posting encompasses three specialized tracks:

  • Central Pharmacy – medication order review, formulary management, drug-interaction monitoring, and clinical documentation.
  • Decentralized / Clinical Specialist – evidence-based medication therapy, patient regimen assessment, clinical rounds, and drug-information research.
  • IV Services / IV Specialist – sterile compounding oversight, parenteral nutrition optimization, chemotherapy regimen review, and hazardous-material management.

Responsibilities

All Clinical Pharmacists

  • Monitor drug therapies for interactions, contraindications, allergies, and appropriateness of drug and dose.
  • Interpret and transcribe medication orders (verbal and written) into computerized patient medication profiles.
  • Maintain accurate, complete patient medication profiles and reconcile medications.
  • Document all clinical activities, interventions, and suspected adverse drug reactions.
  • Provide accurate, timely drug information and cost-effective therapeutic alternatives to medical staff.
  • Review and interpret lab data related to therapeutic drug monitoring.
  • Perform all other related duties as assigned.

Central Pharmacy

  • Sustain the formulary by minimizing non-formulary procurements and utilizing therapeutic substitution protocols.
  • Review renally-excreted medications and adjust dosages based on patients’ renal function.
  • Coordinate and supervise direct support personnel.

Decentralized / Clinical Specialist

  • Provide optimal medication therapy for disease prevention and treatment using evidence-based guidelines.
  • Assess patients’ medication regimens to identify, resolve, and prevent medication-related problems, including dosing, pharmacokinetics, antibiotic management, and core-measure compliance.
  • Attend clinical rounds and support graduate medical education through case reviews and didactic activities.
  • Interact with patients, families, and healthcare providers to ensure continuity of care.

IV Services / IV Specialist

  • Oversee the preparation and storage of sterile preparations for inpatients and outpatients in the Outpatient Infusion Unit, ensuring compliance with USP 797 requirements.
  • Manage the preparation, storage, and handling of hazardous materials, including spill response.
  • Optimize parenteral nutrition by reviewing orders for safety and appropriateness, monitoring lab values, osmolarity, and other parameters.
  • Review chemotherapy regimens for indication, dose, route, preparation, and adjuvant medications, including monitoring for drug interactions and toxicity.
  • Oversee the dispensing of sterile products and supervise support staff in IV services.
  • Collaborate with the Inventory Manager to ensure proper stock levels of high-cost items and generate electronic charges for billing.
  • Maintain cleanliness and proper functioning of the clean room, anteroom, and laminar flow hoods.
  • Lead quality improvement processes in IV services and maintain up-to-date knowledge in pharmacotherapeutics, oncology, nutrition, and biologicals.

Requirements

  • Bachelor’s Degree in Pharmacy from an ACPE-accredited school (required).
  • Rhode Island Pharmacist licensure (required).
  • Knowledge of drug information, medication administration practices, pharmacology principles, and pharmacokinetics.
  • Experience with data entry, electronic medical record (EMR) maintenance, and federal/state regulatory requirements.
  • Experience with chemotherapy and sterile product preparation/dispensing (required for IV Specialist track).
  • Ability to perform clinical interventions and strong communication skills.
