The Clinical Pharmacist at St Michael Medical Center plays a crucial role in ensuring safe and effective medication management for patients. Responsibilities
- Review prescriptions for drug interactions, correct dosing, patient allergies, and anything unusual.
- Interpret medication orders and ensure accurate entry and verification into the computer system.
- Check medications prepared by other pharmacy personnel.
- Communicate drug information to patients and healthcare professionals, resolving any drug-related problems.
- Document clinical activities and interventions such as consults, patient teachings, and other patient-specific information.
- Maintain an adequate knowledge base to provide clinical services for all age groups served.
- Stay updated on automated systems to support clinical services.
Requirements
- Bachelor of Science in Pharmacy or Doctor of Pharmacy.
- Licensure as a registered pharmacist by the Washington State Board of Pharmacy.
- Current American Heart Association Healthcare Provider BLS certification.
Preferred
- Advanced Cardiac Life Support Certification.
