Clinical Pharmacist

Duties/Responsibilities

• Provide and oversee chronic disease state management utilizing collaborative drug therapy management (CDTM) for clinic patients (minimum of 1 patient every hour, total of 8-10 patients a day while on clinic days).
• Absess patient, adjust medications, screen for drug interactions and adverse effects, provide disease/medication education, and explore opportunities to simplify &/or decrease the cost of the medication regimen.
• Collaborate with pharmacy team and provider team to ensure access to medications.
• Work to improve the stewardship of the limited healthcare resources including assisting third party billing specialists with drug prior authorizations as needed.
• Collaborate on innovative projects and policies/procedures to improve the quality and cost-effectiveness of pharmacy-clinic operations and prescribing within the organization.
• Precept other pharmacy residents and students.

Qualifications

• Doctor of Pharmacy or BS Pharmacy degree.
• Completion of PGY-1 residency or 3 years clinical experience in ambulatory care setting.
• Current NYS pharmacist license with immunizer credential.
• Current NYS Collaborative Drug Therapy Management credential, or ability to obtain within 3 months of hire.
• Minimum of 1 year experience in primary care pharmacy involving direct, collaborative patient care.
• Experience precepting students and/or residents.
