Responsibilities
- Provide advanced, patient-centered medication management
- Collaborate with physicians and nurses to optimize pharmacotherapy
- Verify medication orders and monitor for safety
- Support antimicrobial stewardship initiatives
- Conduct clinical rounding and participate in quality improvement and research
- Educate patients and staff on medication use and safety
Qualifications & Skills
- Strong knowledge of pharmacotherapy and medication safety
- Excellent communication and collaboration abilities
- Experience in clinical rounding and medication reconciliation
- Commitment to patient-centered care and continuous improvement
