Job Description

The Clinical Pharmacist is responsible for providing safe and appropriate pharmacy services to Option Care patients. This role involves determining patient suitability for home care, designing and monitoring pharmacy care plans, educating patients and caregivers, and ensuring continuity of pharmaceutical care.

Job Responsibilities

  • Determines the suitability of individual patients for home care.
  • Collects and organizes patient-specific information to assess suitability for home care.
  • Designs, recommends, monitors, and evaluates patient-specific pharmacy care plans.
  • Modifies pharmacy care plans based on evaluation of monitoring data.
  • Provides medication-use education to Option Care staff, other professionals, and patients/caregivers.
  • Ensures continuity of pharmaceutical care to and from the home and other patient-care settings.
  • Manages the use, maintenance, and troubleshooting of medication administration equipment.
  • Provides concise, applicable, and timely responses to requests for drug information.
  • Manages the use of investigational drug products according to established regulations.
  • Manages workload and organizes workflow, prioritizing tasks within available resources.

Basic Education and/or Experience Requirements

  • Graduate of an accredited School of Pharmacy.
  • Current, active Pharmacy license as granted by the State Board of Pharmacy.
  • One of the following:
    • At least one year of experience in sterile IV admixture procedures.
    • Knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM).
    • Completion of an ASHP accredited Pharmacy Residency Program.
    • Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
    • Equivalent experience as approved by the VP of Pharmacy and/or SVP of Clinical Services.

Basic Qualifications

  • Experience providing customer service to internal and external customers, including meeting quality standards for services and evaluating customer satisfaction.
  • Basic level skill in Microsoft Word (opening a document, cutting, pasting, aligning text, selecting font type and size, changing margins and column width, sorting, inserting bullets, pictures, and dates, using find and replace, undo, spell check, track changes, review pane, and printing functions).
  • Basic level skill in Microsoft Excel (opening a workbook, inserting a row, selecting font style and size, formatting cells as currency, using copy, paste, and save functions, aligning text, selecting cells, renaming a worksheet, inserting a column, selecting a chart style, inserting a worksheet, setting margins, selecting page orientation, using spell check, and printing worksheets).
  • Basic skill level in Microsoft PowerPoint (inserting, rearranging, hiding, and deleting slides, navigating between slides, increasing list level, adding, centering, and editing text, changing views, inserting a table or a note, moving objects, printing outline view, and running a slide show).

Preferred Qualifications & Interests (PQIs)

  • Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
  • Experience in home care, hospital, or other clean room setting.

Management Requirements

  • At least 1 year of experience in indirect management of team members, including assisting in the development, training, and assignment of work/projects to other team members.

Pay Range

$107,330.80 - $178,899.67
