Position Details
Join Mercy and play a vital role in ensuring safe, effective, and evidence-based medication therapy for our patients. As a Clinical Pharmacist, you will partner with physicians, nurses, and other healthcare professionals to optimize patient outcomes while serving as a trusted medication expert throughout the organization.
What You'll Do
- Deliver Exceptional Clinical Care
- Review patient records and medication orders to ensure safe, appropriate, and effective drug therapy.
- Evaluate patient conditions and develop individualized medication treatment plans.
- Monitor therapeutic outcomes and make recommendations to improve patient care.
- Identify and prevent adverse drug events, medication errors, and drug interactions.
- Provide medication therapy recommendations regarding dosing, scheduling, routes of administration, contraindications, and formulary alternatives.
- Consult with physicians, nurses, and patients regarding medication therapies and treatment plans.
- Assess treatment outcomes and adjust recommendations as appropriate.
- Maintain accurate and timely documentation of patient care activities and medication plans.
Serve as a Clinical Resource
- Act as a trusted drug information resource for healthcare providers and patients.
- Provide guidance on medication availability, side effects, incompatibilities, formulary considerations, and evidence-based practices.
- Maintain advanced clinical knowledge and specialty expertise relevant to assigned areas.
- Support Medication Therapy Services and collaborate with prescribers to optimize patient outcomes.
Collaborate Across the Healthcare Team
- Partner with physicians, nurses, and interdisciplinary teams to deliver high-quality patient care.
- Participate in quality assurance initiatives, medication use evaluations, and adverse drug reaction reporting.
- Work closely with pharmacy operational staff to support departmental goals and patient care initiatives.
- Serve on committees and contribute to organizational and departmental improvements.
Educate and Mentor
- Precept pharmacy students and residents.
- Support staff competency development and training initiatives.
- Present educational programs, in-services, and clinical lectures.
- Promote continuous learning and professional development throughout the organization.
Support Operational Excellence
- Evaluate prescriptions and medication orders for clinical and operational concerns.
- Participate in investigational protocols as needed.
- Complete required inspections, floor stock checks, expiration date reviews, and regulatory documentation.
- Ensure compliance with accreditation, licensing, and regulatory standards.
- Demonstrate fiscal responsibility and effective resource utilization.
Utilize Advanced Pharmacy Technology
- Work proficiently within pharmacy and clinical information systems, including Epic.
- Utilize drug information resources such as Micromedex.
- Operate pharmacy automation technologies including Omnicell, McKesson Automation, ScriptPro, Baxa Systems, and related platforms as needed.
