About the role
The Clinical Pharmacist I provides high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life.
Responsibilities
- Provides clinical pharmacy services to patients in collaboration with the healthcare team.
- Collaborates with physicians, nurses, and other healthcare providers to develop and implement medication therapy plans.
- Monitors and evaluates patient medication therapy to ensure safety, efficacy, and appropriateness.
- Participates in medication reconciliation and medication management processes.
- Assists in the development and implementation of policies and procedures related to medication management.
- May participate in research activities related to medication therapy.
Requirements
- PharmD in Doctorate of Pharmacy or Bachelor's Degree in Bachelor of Pharmacy.
- Upon hire: Washington Pharmacist License.
- Basic Life Support (BLS) (Facility Managed) - where applicable.
- Board of Pharmacy Preceptor License - when eligible and where applicable.
Qualifications
- Acute care hospital pharmacy experience, 1 year or equivalent experience.
- Completion of ASHP-accredited PGY1 residency.

Schedule
Night Shift, working 10 hour night shifts, with 7 nights on and 7 nights off.
