How you’ll make an impact in this role
Enhance Patient Outcomes: Pro-actively manage chronic diseases (e.g., diabetes, hypertension) to prevent complications and improve overall quality of life.
Optimize Pharmacotherapy: Perform comprehensive medication management, interpret labs, and safely adjust dosages under collaborative practice agreements.
Boost Medication Adherence: Identify financial or insurance barriers and provide targeted education to streamline therapy and ensure access.
Support the Care Team: Collaborate directly with providers to streamline workflows and ease provider burnout.
Minimum Requirements You’ll Need
Licensure / Certification / Registration: Pharmacist credentialed from the Oklahoma State Board of Pharmacy required.
Education: Bachelor's or advanced degree in Pharmacy required.
Additional Preferences We're Seeking
Pharmacist licensure by the state of Kansas prior to or within 6 months of hire date preferred
Board Certified Ambulatory Care Pharmacist credential preferred
Completion of a PGY-2 Ambulatory Care Pharmacy Residency in lieu of Ambulatory Pharmacist experience
