Clinical Pharmacist Ambulatory Services
About the Role
As our Pharmacist, you’ll help doctors and nurses safely prescribe and dispense medications to help patients feel better and recover from illness or injury. Every day you will meticulously review every prescription in your queue for safety, critically assessing for potential drug interactions, correct dosing, patient allergies, or any irregularities. You'll then authorize prescriptions for the pharmacy technician, performing a final check before dispensing medication to the nursing station, ensuring expert oversight in every process.
To succeed in this role, you must enjoy learning new concepts and researching information in real-time to provide solutions for real people. You should also continually look for ways to go above and beyond for your patients, delivering an exceptional experience along with exceptional care.
Responsibilities
- Participate in the evaluation and management of diseases and health conditions in collaboration with other health care providers
- Prescribe pharmaceutical agents as outlined in the approved collaborative drug therapy management agreement, including initiating, adjusting, and discontinuing drug therapy
- Order and interpret drug therapy-related tests
- Ensure patients have access to and receive safe, efficacious, and cost-effective medication therapy
- Work collaboratively to resolve medication access and adherence issues
- Address medication-related issues to support care coordination and transitions of care
Requirements
- Graduation from an accredited school of pharmacy (Doctor of Pharmacy preferred)
- Washington state Pharmacist license
- One of the following:
- Completion of three (3) years of practice experience with at least 50% of time spent in ambulatory care activities, or
- Completion of a PGY2 Ambulatory Care Pharmacy residency, or
- Completion of a PGY1 residency plus one (1) year of practice experience with at least 50% of time spent in ambulatory care activities

Pay
$69.30 - $94.00 per hour
