Clinical Pharmacist, Ambulatory Care

Part-time (40 hours per pay period), salaried position with flexible Monday through Friday daytime hours.

About the role

Clinical Pharmacist, Ambulatory Care within the Trinity Health Medical Group primary care, Patient-Centered Medical Home (PCMH) offices in Grand Rapids, Michigan. Responsibilities include disease management under institution-approved collaborative practice agreements (diabetes, hypertension, hyperlipidemia, asthma, COPD, anticoagulation, and others), referral-based Comprehensive Medication Review with recommendations to physicians, and daily ad-hoc consultations to improve pharmacotherapy effectiveness, safety, and cost. Additional activities include coverage for the Anticoagulation Management Service (approximately 1 day per week) and multidisciplinary initiatives targeting high-risk patients or populations (e.g., depression, frail elderly; approximately ½ day per week). Participation in training Trinity Health Grand Rapids PGY-1 & PGY-2 Pharmacy residents and PharmD students from Ferris State University College of Pharmacy, as well as involvement in medical group and health-system committees, is expected.

Responsibilities

• Provide drug therapy management and assessment for outpatient patients.
• Identify and resolve potential and actual drug-related problems.
• Assist physicians in designing dosing regimens based on patient-specific parameters.
• Communicate and collaborate with physicians and other clinicians to develop drug therapy policies, protocols, and guidelines.
• Support the Pharmacy & Therapeutics Committee, including preparing formulary drug reviews, medication usage evaluations, and policies/guidelines for drug therapy.
• Participate in performance improvement initiatives and research as appropriate.
• Engage in departmental, multidisciplinary, hospital, and health-system committees as leader, member, or consultant.
• Train, educate, precept, and evaluate pharmacy students and/or residents.
• Provide education and information on pharmaceutical care and drug therapy to patients, pharmacy staff, physicians, nurses, students, and other clinicians.
• Perform other related duties such as quality assurance reports and preparation for regulatory inspections.

Requirements

• Doctor of Pharmacy degree (PharmD) from an ACPE-accredited school/college of Pharmacy (required).
• PGY-1 Residency (required).
• PGY-2 Ambulatory Care Residency (strongly preferred) or commensurate practice experience in ambulatory care with direct pharmacotherapy management responsibilities.
• Board of Pharmacy Specialties (BPS) Specialty Certification in Pharmacotherapy or Ambulatory Care (preferred; or eligible with certification within 1 year of hire).
• Licensed (pharmacist and controlled substance) in the State of Michigan, or eligible.
