About the role

The clinical pharmacist provides comprehensive medication management services that integrate clinical services, order management, patient/staff education and research. Clinical pharmacists are responsible for improving the patient’s experience, safety, and achieving positive clinical and financial outcomes.

Responsibilities

• Provides patient care according to the laws and regulations governing pharmacists and the practice of pharmacy by the appropriate state and federal agencies.
• Works relatively independently, and within the limits of established pharmacy/ambulatory care practice site’s policies and procedures, as a member of the patient care team.
• Responsible for providing medication management services in a collaborative manner that consists of reviewing patient medication history, educating patients on drug therapy, establishing desired outcomes, developing/ modifying therapeutic plans, determining alternatives for achieving outcomes, developing drug monitoring plans, monitoring patient outcomes, documenting medication related issues in electronic health record, interpreting laboratory data, reviewing patient medication profiles, monitoring pharmacokinetics of drug therapy, monitoring for potential adverse drug – drug, drug-lab, drug-disease state, and drug - food interactions.
