About the role

The Clinical Pharmacist is a specialist position responsible for working with physicians, care managers, and other clinical staff, focusing on patient outcomes while maintaining cost. They provide therapeutic assessments and disease management, fostering teamwork and ensuring high-quality patient care.

Responsibilities

  • Directs and implements clinical pharmacy services.
  • Provides clinical consultation to the Medical Staff and other health care providers.
  • Communicates and documents all activities and interventions.
  • Fosters teamwork and ensures delivery of high-quality patient care.
  • Performs staff duties as needed.
  • Collaborates with all health care providers to resolve concerns and questions.
  • Develops and implements treatment guidelines, pathways, protocols, and other drug information reports.
  • Participates in clinical committees and maintains awareness of regulatory standards.
  • Supports 340B compliance and manages prior authorizations and appeals.
  • Ensures compliance with ACHC and URAC standards and participates in audit and quality improvement projects.

Qualifications

  • Hospital Pharmacy: Pharm.D. Degree with 2 years of experience in inpatient pharmacy hospital/clinical capacity or completion of Pharmacy Residency.
  • Specialty Pharmacy: Pharm.D. Degree with 2 years of experience in retail / specialty pharmacy, hospital/clinical capacity or completion of Pharmacy Residency.
  • Currently licensed or registered to practice pharmacy in Louisiana.
  • Board Certification in applicable specialty area preferred.
