Clinical Performance Coordinator

Panels and Patient Engagement

• Identify and outreach to unaligned or attributed patients to establish primary care relationships.

Transitions of Care

• Monitor daily admission, discharge, and transfer (ADT) reports.

Care Quality Gaps

• Identify care gaps using payer reports, registries, and EHR tools.
• Partner with clinical staff to ensure completion of required screenings, labs, and follow-ups.

Annual Wellness Visits (AWVs)

• Proactively identify eligible Medicare patients for AWVs.

Billing and Documentation Support

• Ensure accurate documentation supports value-based billing opportunities.

Data Tracking and Reporting

• Maintain dashboards and reports on panel performance, TOC visits, AWVs, and care gap closure.

Care Management Collaboration

• Be the central “hub” for the providers for care coordination concerns.

Experience

Required: 2-4 years of relevant experience

Experience in healthcare operations, population health, care coordination, or medical office workflows. Experience working in primary care or value-based care environments strongly preferred. Strong understanding of population health and value-based care models.

Education

Preferred: Associate's Degree

Licenses and Certifications

• Preferred:
• - Basic Life Support
• - Certified Medical Assistant (AAMA)
