About the role
The Clinical Performance Coordinator plays a critical role in supporting primary care teams by optimizing patient panel management, improving patient engagement, and ensuring high-quality, value-based care delivery. This position focuses on outreach to unaligned patients, coordination of transitions of care, closure of quality care gaps, and driving completion of Annual Wellness Visits (AWVs). The specialist partners closely with providers, care managers, and administrative staff to improve outcomes, patient experience, and reimbursement performance.
Responsibilities
- Optimize patient panel management to enhance care delivery.
- Improve patient engagement through targeted outreach.
- Coordinate transitions of care for patients.
- Close quality care gaps to ensure high-value care.
- Drive completion of Annual Wellness Visits (AWVs).
- Collaborate with providers, care managers, and administrative staff to improve outcomes and patient experience.
Requirements
- 2-4 years of relevant experience.

Pay
$19.10 to $28.65 per hour, based on experience.
