Clinical Outcomes Coordinator
Job Summary
Responsible for Value Based Purchasing data and coordination. Coordinates National Patient Safety Goals, readmission work, Patient Safety Indicator work, and mortality team. Works with all levels of leadership and staff and collaborates with Risk Management, Accreditation, and Performance Improvement teams for Mission Health System. Facilitator for clinical quality initiatives.
Qualifications
- Maintains awareness and knowledge for VBP, NPSGs, and PSIs
- Captures data and information from medical records and uses critical thinking to establish facts and draw valid conclusions
- Knowledge of patient safety principles, national/local patient safety and accreditation standards and clinical best practices
- Basic knowledge of human factors theory, systems improvement techniques and Performance Improvement methodologies
- Proficiency in Microsoft suite of tools (to include Outlook, Word, Excel and PowerPoint)
- Organizational skills, including ability to multitask and manage multiple projects with attention to detail
- Excellent written, oral, and intrapersonal skills; ability to work effectively independently and in teams; ability to serve as change agent
Preferred Qualifications
- In-depth understanding of human relations, group dynamics, change theory, and improvement methodologies
