About the role

NuvoAir Medical is seeking a dynamic and passionate individual to join our growing team as a Chronic Disease Manager. The ideal candidate will have a strong background in healthcare management, experience with chronic respiratory diseases, and a proven track record of managing complex patient cases.

Responsibilities

  • Develop and implement comprehensive care plans for patients with chronic respiratory diseases
  • Monitor patient progress and adjust care plans as necessary
  • Communicate regularly with patients and their families to ensure they understand their condition and treatment plan
  • Collaborate with multidisciplinary teams to coordinate care and address patient needs
  • Conduct regular assessments to identify patient needs and opportunities for improvement
  • Stay updated on the latest research and best practices in chronic disease management

Requirements

  • Bachelor’s degree in Healthcare Administration, Nursing, Respiratory Therapy, or related field
  • At least 5 years of experience in chronic disease management or related field
  • Proven ability to manage complex patient cases and develop effective care plans
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Experience with remote patient monitoring systems and electronic health records

  • Experience with telehealth platforms and remote patient monitoring tools
  • Excellent written and verbal communication skills
  • Ability to work effectively with a diverse range of stakeholders
