Clinical Nurse Coordinator - Infectious Disease Clinic

About the role

Provides specialized care to specific patient populations to optimize care and experience. Coordinates the plan of care and acts as a liaison between providers, departments, other healthcare professionals and schedulers to coordinate testing, scheduling, referrals etc.

Responsibilities

  • Provides hands on assistance according to the department, patient and provider needs, including but not limited to assisting with procedures, performing tests, preparing or administering patient medications, obtaining specimens.
  • Act as first contact for patients, providing triaging and medical advice where applicable.
  • Captures and coordinates the plan of care and acts as a liaison between providers, departments, other healthcare professionals and schedulers to coordinate testing, scheduling, referrals etc.
  • Makes sure patient’s appointments, testing and labs necessary for the patient population are managed.
  • Aids in obtaining prior-authorizations for the patient population.
  • Prepares educational material for patient use.
  • Educates and counsels patients concerning medical condition, medication or treatment.
  • Provides specialized nursing assistance in research efforts when applicable.
  • Provides advice, education and consultation services to nursing staff of any unit that care for the same patient population.
  • Attends conferences or educational opportunities to ensure up to date knowledge.

Requirements

  • Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
  • State criminal background check and Federal (if applicable), as required for regulated areas.
  • Obtain certification in Basic Life Support within 30 days of hire date.

Qualifications

  • Two years’ experience as an RN OR five years’ experience as an LPN required.
  • Bachelor's Degree in nursing(BSN) preferred.
  • Preferred certifications relevant to the specialized care they will be providing.

Skills

  • Strong communication skills.
  • Ability to manage multiple tasks simultaneously.
  • Knowledge of medical terminology and procedures.
  • Ability to work independently and as part of a team.
