Responsibilities
- Affirms physical, emotional, social, and spiritual needs and evaluates patients' adaptation to health changes
- Develops a plan of care based on the patient's disease, symptoms, and response to treatment
- Educates patients, families, and caregivers on disease processes, treatment, side effects, and symptom management
- Collaborates with the interdisciplinary team to ensure continuity of care
- Refers patients, families, and caregivers as appropriate to other clinical resources/professionals such as social work, home health care, clinical nutrition, etc.
- Implements, evaluates, and modifies the plan of care
- Integrates research and evidence-based knowledge into clinical practice
Additional Responsibilities
- Direct Patient Care
- Engages in meaningful patient follow-up and care coordination, including post-procedure, medication side effects, and new patient orientation
- Supports caregivers by providing support and answering their questions
- Assists with in-clinic procedures, including set up, consent, time out, and patient positioning
- Assists with prescription refills and medication questions
- Reviews results and coordinates with the research team for patients on trial
- Facilitates external referrals for ongoing rehabilitation needs
- Manages Epic Rehab Medicine Clinical Support InBasket inquiries from patients, staff, or other providers
- Communicates and collaborates with the research team
Qualifications
- Graduated from an accredited school of nursing
- Current Washington State Registered Nurse (RN) license or current NLC multistate license (MSL)
- BLS AHA Health Provider Card (or equivalent course, such as by the American Red Cross)
- Bachelor of Science in Nursing (BSN) preferred
- OCN (Oncology Certified Nurse) or BMT RN certification preferred
- Two years of oncology nursing experience preferred
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- OCN (Oncology Certified Nurse) or BMT RN certification
- Previous telephone triage experience
- Previous outpatient clinic experience
