Make a Difference Every Day as a Clinical Nurse in Community Services.

Responsibilities

  • Allocated a client portfolio and have the opportunity to lead and inspire the frontline clinical team through the provision of guidance and mentorship.
  • Effective decision making and clinical experience where you will provide exceptional care whilst responding to clinical nursing issues and concerns.
  • Review Client Care And Care Plans Promoting Independence & Quality Of Life, Making a Real Difference To The Quality Of Care We Deliver.
  • Development, implementation, monitoring and review of care plans
  • Comprehensive and accurate assessment of residents
  • Aiding with the orientation of new staff and ongoing professional development
  • Support in ensuring the service meets all accreditation standards

Requirements

  • An experienced clinician, with considerable experience within an acute or aged care environment.
  • An advocate for person-centred care, leading by example through exceptional communication and customer service.
  • Bachelor of Nursing, or equivalent qualification
  • Current AHPRA registration
  • Extensive general nursing experience and demonstrated skills reflective of clinical experience
  • Ability to liaise with the care team and to provide co-ordinated care
  • Confidence to manage conflict, pressures and competing priorities
  • Experience working in a multidisciplinary team with ability to provide leadership, support and mentorship to other team members
  • Previous community nursing experience and knowledge in community funding is highly desirable

Qualifications

  • Bachelor of Nursing, or equivalent qualification
  • Current AHPRA registration
