About the Role

Provides high-quality, patient-centered outpatient care across various clinical settings, supporting multiple ambulatory practice areas such as specialty clinics and procedural areas. The role involves assessing, planning, implementing, and evaluating patient care, educating patients and families, and promoting safety and quality compliance.

Responsibilities

• Develops and documents individualized care plans in collaboration with the healthcare team and patients.
• Assesses physical, psychosocial, and emotional needs, considering patient preferences and barriers to care.
• Supports patient safety, quality standards, and professional development activities.
• Addresses social determinants of health to improve health outcomes.

Requirements

• Minimum of 5 years RN experience, preferably in Critical Care, ED, or Infusion settings.
• RN license required; BSN preferred.
• Strong critical thinking, communication, and flexibility skills are essential.
• Ability to adapt to diverse clinical environments and work independently.
