Clinical Navigator Oncology Center FT Days
Job Summary
The Oncology Navigator RN is a registered professional nurse with clinical oncology knowledge who works in multiple departments to provide individualized culturally sensitive assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process, the role provides education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the care continuum.
Qualifications
- BSN
- Graduate of an NLN accredited school of nursing required
- A minimum of two (2) years’ Nursing experience, with at least one (1) year in Oncology
- Preferred: MSN Degree, Knowledge of standard of care treatment for Oncology Patients, One (1) year customer service experience in a healthcare setting, Two (2) years of Care Coordination in an Ambulatory setting
- Certifications and Licenses Required: Active NJ RN license or compact RN license with NJ endorsement, BLS from the American Heart Association, Navigation Certification within (3) three years of employment
Scheduling Requirements
Day Shift, 8:30a - 4:30p (variable and not guaranteed), Hybrid work schedule, Full-Time, 37.5 hours, Monday – Friday
Essential Functions
- Patient-Centered Navigation: Proactively and effectively guides and monitors patients and families through complex cancer care systems, ensuring access, equity, and continuity of care across settings.
- Resource Coordination & Advocacy: Assesses clinical and non-clinical needs connecting patients to resources (e.g., financial, transportation, psychosocial), while proactively advocating for the patient.
- Education & Engagement: Educates patients, caregivers, and staff on treatment adherence, follow-up, and culturally sensitive care practices to promote informed decision making and compliance.
- Team Collaboration & Communication: Collaborates with the navigation team, and multidisciplinary team to support care plans, demonstrating strong communication, and problem-solving skills in a self-motivated patient centered care setting.
- Professionalism & Documentation: Exhibits compassion, cultural humility, and integrity while maintaining accurate documentation using standardized tools and charting guidelines to track patients and outcomes. Tracks and documents time spent in accordance with PIN billing requirement, demonstrating support of eligible navigation services provided. Participate in quality improvement, accreditation, and program evaluation initiatives.

- Clinical Oncology Knowledge – Demonstrate knowledge of cancer diagnoses, treatment modalities, symptom management, and the oncology care continuum.
- Patient Assessment and Care Coordination – Assesses patient needs, identify barriers to care, and coordinate timely resources for access across the continuum.
- Communication Skills – Communicate effectively with patients, families, providers, and interdisciplinary teams using compassionate, patient-centered approaches.
- Patient Education and Shared Decision-Making – Provides education tailored to health literacy and cultural needs, supporting informed treatment decisions.
- Advocacy and Resource Navigation – Advocates for patients and families while connecting them with financial, psychosocial, transportation, and community resources.
- Critical Thinking and Problem-Solving – Analyzes complex situations, identifies solutions, and independently addresses barriers impacting care delivery and outcomes.
- Cultural Competence and Health Equity – Demonstrates cultural humility and promotes equitable access to care for diverse patient populations.
- Documentation and Technology Proficiency – Accurately documents assessments, interventions, and outcomes in the electronic medical record and utilizes data to support quality care and program goals.
