Responsibilities
As a Clinical Navigator, you will:
- Meet people at their worst of times and share success stories.
- Facilitate the referral conversion by working collaboratively with various teams.
- Develop and maintain relationships with stakeholders including prospective patients, family members, physicians, nurses, discharge planners, case managers, and payer representatives.
- Provide feedback and recommendations for program development and quality improvement initiatives related to customer service, the referral and admission process, and patient and referral source satisfaction.

Qualifications
Qualifications include:
- Education: Degree in a health-related field from an accredited college or university preferred.
- Licensure: Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
- Prior experience: Must have a minimum of two years’ experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.
