Responsibilities
- Aid in the admission process for patients admitted to the inpatient unit, including care plan initiation, patient education, and documenting home medication history in the EMR.
- Coordinate the completion of EMR and paperwork for patients transitioning from inpatient status to Swing Bed status.
- Assist with the admission process to Swing Bed status, offering solutions to any admission challenges or problems.
- Complete, document, and submit Minimum Data Set (MDS) and daily physical assessments via the submission portal at required intervals.
- Apply knowledge of healthcare reimbursement methodologies and cost structures to evaluate and support appropriate Swing Bed placement decisions.
- Coordinate interdisciplinary team efforts along the continuum of care to better meet the needs of patients within the Swing Bed program.
- Function as a contact/resource person for patients and their families, assisting with coordination of clinical rehabilitation disciplines and distribution of information.
- Facilitate weekly interdisciplinary meetings with physicians, nurses, case management, PT/OT, Speech, and patient families when available.
- Maintain ongoing knowledge of CMS and CIHQ regulations and guidelines applicable to Swing Bed programs to ensure regulatory compliance and program integrity.
- Represent the Swing Bed program on committees, councils, and task forces.
Requirements
- Bachelor of Science in Nursing (BSN) required.
- Master's degree preferred.
- Three (3) years minimum experience in clinical specialization required.
- Two (2) years of post-acute care experience required.

- Excellent prioritization and communication skills.
- Demonstrated ability to implement, support, and effectively monitor change.
- Ability to analyze reimbursement and productivity.
- Excellent communication skills facilitating collaboration with various departments such as Finance, Physical and Occupational Therapy, Speech, and Case Management.
