Role Overview

The Home Health Patient Care Manager oversees and coordinates clinical services, supervising an interdisciplinary team to ensure high-quality, individualized patient care in accordance with physician plans and legal regulations.

Key Responsibilities

• Provide clinical services within scope of practice, aligned with the plan of care and team collaboration
• Manage patient referrals, clinician assignments, and evaluate orders to initiate care
• Coordinate patient benefits, medical necessity, and insurance approvals
• Assess patient needs continuously, ensuring care meets eligibility criteria and physician directives
• Review assessments and plans of care daily, offering recommendations to clinicians

Qualifications And Skills

• Unrestricted RN license in the practice state
• CPR certification
• Valid driver's license, vehicle insurance, and access to transportation
• Ability to adapt to diverse home environments and serve clients regardless of background
• Preferred: home care experience, independent work skills, strong communication and organizational abilities
