About the role
The Clinical Manager, Palliative Care coordinates and manages all day-to-day business affairs and clinical activities related to the administration of the palliative division. Ensures clinical care delivery goals are met and exceeded, coordinates clinical responsibilities for the market, and oversees scheduling, frequency adherence, clinical coordination, medical record maintenance, and marketing functions for a team of patients and clinicians.
Responsibilities
- Manage incoming calls from partners and patients.
- Ensure visit frequency compliance, schedule PRN visits, and coordinate post-hospitalization follow-up with patients, families, and partners.
- Facilitate orders for labs, durable medical equipment (DME), and imaging; coordinate results with Nurse Practitioners (NPs).
- Enter telephone encounters and routine medication reconciliation in partner EMR.
- Ensure transitions of hospice-eligible referrals to Continuing Care (CTC).
- Identify home health providers that can accept referrals from Palliative Care Clinicians (PCC).
- Coordinate with local partner offices for lab drop-off.
- Adhere to and develop company policies, procedures, and departmental goals.
- Act as a role model, maintain a positive and respectful attitude, and communicate regularly with supervisors about department issues.
- Demonstrate flexible time management, prioritize workload, and consistently report to work prepared.
- Participate in administrative meetings, committees, and special projects; recommend improvements to policies and documentation.
- Ensure compliance with local, state, and federal regulations, as well as company policies.
- Coordinate and direct all day-to-day clinical activities of the palliative program.
- Serve as a contact for patient calls and coordinate appropriate follow-up.
- Support the referral process by obtaining admission and insurance information for timely patient admission.
- Schedule Nurse Practitioners with geographical considerations for efficient time and cost management.
- Monitor risk scoring and ensure visit frequency adherence as indicated by Global Alerts.
- Reschedule patients to meet required visit frequencies with NPs, RNs, or other clinical staff.
- Track emergency department visits and hospitalizations; document details in the hospitalization tracker.
- Schedule post-hospitalization/ER visits with appropriate staff, including field and remote teams.
- Support NPs with submitting orders for DME, imaging, and labs; track and communicate outstanding results.
- Upload lab and imaging results to the EMR and document results in telephone encounters.
- Update medication reconciliation in the partner’s EMR as needed.
- Assist with credentialing documents for payors and maintain a credentialing log.
- Manage daily clinical records functions, ensuring compliance with state, federal, and company regulations.
- Coordinate and prepare agendas for interdisciplinary team (IDT) meetings.
- Oversee the home health and hospice referral process, ensuring timely acceptance of referrals.
- Establish and maintain professional relationships with referral sources, including Gentiva family companies.
- Act as a resource for customers and the community regarding palliative care.
- Maintain high-quality customer service and monitor referral source satisfaction levels.
- Identify opportunities for improved services through family and physician satisfaction surveys.
- Document customer complaints and follow up with the program director.
- Participate in quality assurance and performance improvement activities, including data collection and reporting.
- Implement marketing and promotional initiatives as directed by the Director.
- Adhere to HIPAA privacy practices, business ethics, compliance programs, and company policies.
- Facilitate onboarding and training for new employees and conduct employee evaluations.
Requirements
- Registered Nurse with Home Health, Hospice, and/or Palliative Care experience preferred.
- Bachelor’s degree in Nursing, Business Administration, or related field; or equivalent experience and knowledge in healthcare operations required.
- Master’s degree in business or related field preferred.
- Minimum of 2 years of healthcare administration/operations management required.
- Certified in Hospice and Palliative Care preferred.
- Fluent competence in computer and smartphone navigation.
- Working knowledge of Microsoft Outlook, Word, Excel, and PowerPoint.
- Reliable transportation, current automobile insurance, and valid driver’s license required.
