Pay

$95,000 to $109,000 per year, plus a $10,000 sign-on bonus.

Responsibilities

  • Recommends employment decisions (hire, fire, promote, or change status) to the director of operations
  • Oversees overall quality of patient care and clinical performance of nurses and aides
  • Organizes clinical operations for the care center
  • Reviews service requests and determines patient eligibility/suitability for home care services
  • Coordinates patient referrals and admissions with intake
  • Manages scheduling, payor verification, and precertification requirements
  • Reviews staff documentation to resolve missing, incomplete, or untimely issues
  • Assists clinicians in establishing immediate and long-term patient goals, setting priorities, and developing the Plan of Care (POC)
  • Regularly evaluates patient needs and revises the plan of care in collaboration with clinicians
  • Provides direct patient care on an infrequent basis during emergencies
  • Participates in the on-call process
  • Trains staff to ensure quality care, compliance, and fiscal responsibility
  • Investigates and resolves patient, family, or physician concerns

Requirements

  • Associate or bachelor’s degree in nursing
  • Current RN license specific to the state(s) of assignment
  • One year of clinical RN experience in home health or hospice
  • One year of RN management experience
  • Current CPR certification
