Responsibilities

• Meet with patients in hospitals or nursing facilities to discuss services and transition them to home health or hospice care.
• Act as a clinical liaison to internal Branch Directors, assessing patient complexity and validating that needs can be met at home.
• Coordinate home health services with physicians, including taking written and verbal orders.
• Partner with Account Executives to provide clinical education to referral sources.
• Review and assist with hospital hold lists; participate in sales meetings and case conferences.

Qualifications

• Associate’s or Bachelor’s degree in nursing with an active LVN/LPN license.
• 1–3 years of relevant experience.
