Responsibilities

Affords patients the opportunity to navigate post-acute care processes with the aim of enhancing patient outcomes and improving referral source satisfaction.

Utilizes evidence-based clinical guidelines, preventative strategies, protocols, and relevant metrics in the creation of transition plans that prioritize patient-centered care, aiming for quality and efficiency in post-acute care delivery.

Fosters strategic partnerships with health systems, hospitals, inpatient facilities, physicians, physician groups, and executive-level entities.

Requirements

• Must be a graduate of an approved nursing, therapy, or social work program.
• Must be licensed in the state of practice.
• Must have at least two (2) years of field experience.
• Must demonstrate knowledge of federal, state, and local regulations governing Medicare-certified home health and hospice operations.
• Must possess basic technology skills, including the ability to operate a mobile device.

Qualifications

• Education and experience, essential: Graduate of an approved nursing, therapy, or social work program. Licensed in the state of practice. Two (2) years of field experience.
• Education and experience, preferred: Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), or Licensed Medical Social Worker (LMSW). Three (3) years of field experience. Previous experience in home health or healthcare sales.
