Responsibilities
- Advocate for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs
- Serve as a resource for healthcare professionals through fostering relationships within medical facilities
- Coordinate care plans with physicians and families
- Facilitate patient transfers and address barriers to accessing specialized post-ICU care
Full-time
- Monday-Friday 8-5 PM
Pay
- Competitive Salary ($70,000 - $90,000 depending on experience)
- Plus uncapped monthly bonus incentives

Key Responsibilities
- Work with physicians, discharge planners, and patients' families to plan the best continuum of care after an ICU or post-surgical stay by utilizing your strong clinical background, payer knowledge, and problem-solving skills.
- Perform clinical assessments of referred patients, then counsel with patients' families to assist them in making the best care decisions for their loved one.
- Partner with hospital case managers and physicians to create a smooth pre-admission and transfer process through removing payer-related and operational barriers for patients getting into our highly effective level of care.
Required Skills
- Registered Nurse (RN) or Respiratory Therapist (RRT) License (other designations like LPN, PT, OT, etc., upon further review).
- Ability to complete clinical assessments of medically complex patients.
Preferred Skills
- Demonstrated success in patient advocacy.
- Superior communication and relationship-building skills.
- Prior healthcare marketing or business development experience is preferred, but extensive training is provided on this piece.
