Clinical Liaison Home Health

Responsibilities

• Affords patients with guidance through post-acute care processes.
• Develops transition plans that align with evidence-based clinical guidelines, preventative standards, and other relevant metrics.
• Engages in strategic partnerships with health systems, hospitals, inpatient facilities, physicians, and physician groups.

Qualifications

• Graduate of an approved nursing, therapy, or social work program.
• Licensed in the state of practice.
• At least 2 years of field experience.
• Familiarity with Medicare certification regulations and compliance.
• Basic technological proficiency, including mobile device operation.

Requirements

• Valid state driver license.
• Automobile liability insurance as required by law.
• Dependable transportation in good working condition.
• Safety in operating a vehicle under various weather conditions.
